Do Urgent Cares Do Ultrasounds? What Stays True No Matter the Clinic
Some urgent care centers do ultrasounds; most do not. Where the service exists, it is almost always point-of-care ultrasound (POCUS): a focused bedside scan performed and interpreted by the treating clinician, usually inside a few minutes, to settle one question. Is there a clot in this vein. Is the pregnancy inside the uterus. Is this kidney obstructed. The American College of Emergency Physicians, in the policy statement "Ultrasound Guidelines: Emergency, Point-of-Care, and Clinical Ultrasound Guidelines in Medicine," names urgent care explicitly among the settings where such scanning occurs and treats it as a category separate from consultative imaging. The other service, a formal radiology ultrasound, works differently: a registered sonographer completes a full protocol, and a radiologist who never meets the patient issues a written report afterward. Both get called "an ultrasound." They answer different questions, on different clocks, at different prices.
That difference is the part that holds no matter which clinic a patient walks into. Whether the machine is there at all, what it costs, who is holding the probe — all of that swings wildly by location.
How many urgent cares have an ultrasound machine?
Far fewer than have X-ray. Experity, the urgent care software company, analyzed its client clinics for visits between July 1, 2018 and June 30, 2019. Ninety-two percent documented at least one radiology CPT code, though only 10.7 percent of all visits included a radiology code. Of the visits that did, X-rays accounted for 88 percent, rising to 97 percent once EKGs are set aside. CT, ultrasound and MRI together made up less than 3 percent.
Adoption has been slow enough that The Journal of Urgent Care Medicine modeled whether the equipment pays for itself. Weissert, Russell and Havryliuk sampled 10,000 adult urgent care encounters and judged 922 of them, or 9.2 percent, appropriate for a POCUS assessment, with lung ultrasound the most frequent indication. Their break-even projection landed at 16 patients per day against an industry average daily volume of 43.
"POCUS adoption would enhance clinical operations while being revenue neutral or positive in urgent care centers with typical volumes," said Josh Russell, MD, MSc, chief medical officer of UCP Merchant Medicine and a co-author of that analysis.
The same journal notes that adoption clusters in clinics led by emergency-medicine-trained physicians and in centers doing higher-acuity or ultrasound-guided orthopedic work. A center staffed mainly by family medicine is less likely to have it.
Who holds the probe, and what training stands behind it
This is where the two services separate most sharply.
A formal radiology ultrasound is performed by a sonographer credentialed through the American Registry for Diagnostic Medical Sonography. Under ARDMS rules, earning the RDMS credential requires passing the Sonography Principles and Instrumentation physics examination plus a specialty examination in Abdomen, OB/GYN or another approved area within five years of each other, after completing an accredited sonography program or an equivalent education-plus-experience pathway.
A clinician performing POCUS is credentialed by a different route. ACEP's guidelines set a benchmark of 25 to 50 quality-reviewed examinations in each application a clinician wants to use, with an overall target of 150 to 300 examinations, and 10 reviewed procedures for ultrasound-guided work. Skills maintenance is set at a minimum of 25 scans and three hours of continuing education every two years. ACEP wrote those numbers for emergency physicians and states that other specialties may adopt the document if they scan in the manner it describes; there is no urgent-care-specific equivalent from a national body.
Duration follows. MedlinePlus, the National Library of Medicine's patient resource, says of abdominal ultrasound that "most of the time, the test takes less than 30 minutes," and the Cleveland Clinic gives roughly the same figure. A bedside compression scan of one leg or a transabdominal look for a gestational sac is a fraction of that, because it is not attempting the same coverage.
How accurate is a bedside ultrasound? Entirely depends on the question
Accuracy is not a property of the machine. It is a property of the question being asked of it, and the published numbers spread across an enormous range.

The DVT figure carries a caveat the field argues about openly. A separate systematic review of 15 publications covering 2,511 examinations pooled sensitivity at 90 percent and specificity at 95 percent, and found operator experience mattered enormously: 93 percent accuracy for attending emergency physicians against 77 percent for trainees. A Western Journal of Emergency Medicine analysis pushed back harder, arguing that estimates across the dozen-odd studies are inconsistent and that the strongest mid-to-high-90s results, produced by small numbers of expert operators, have not been replicated with emergency physicians as operators. The 2024 meta-analysis found complete compression and whole-leg duplex studies pooled at 100 percent sensitivity. Two-point compression misses roughly 4 percent of clots.
The kidney stone number is the one patients find most surprising. In the multicenter trial led by Rebecca Smith-Bindman, MD, of the UCSF Department of Radiology, 908 patients were assigned to point-of-care ultrasonography, 893 to radiology ultrasonography and 958 to CT. Sensitivity for nephrolithiasis on the first imaging test was 54 percent for point-of-care ultrasound, 57 percent for radiology ultrasound and 88 percent for CT. Specificity ran the other direction: 71, 73 and 58 percent. Roughly four in ten of the point-of-care patients went on to have a CT anyway.
The trial also found why the scan is still worth doing. Among patients who had only one imaging examination, median emergency department length of stay was 5.1 hours in the point-of-care group against 6.4 hours in the radiology ultrasound group and 6.2 hours in the CT group. Serious adverse events, return visits, admissions and pain scores did not differ significantly between groups.
Why a leg scan at urgent care may not be a "duplex" study
Patients sent for a clot check often assume any leg ultrasound is the same study. The billing definitions say otherwise. The AMA's CPT descriptor for code 93970 reads "duplex scan of extremity veins including responses to compression and other maneuvers, complete bilateral study"; 93971 covers the unilateral or limited version. "Duplex" means B-mode imaging combined with Doppler, and a compliant report is expected to document color flow Doppler, spectral waveform analysis, and the compressibility and augmentation response of each named venous segment.
A two-point bedside compression exam checks whether the common femoral and popliteal veins collapse under probe pressure. That is a real diagnostic maneuver, and many handheld units now include color Doppler. It is not the same document as a complete duplex study, and it does not survey the calf.
Credentialing splits along the same line. The Intersocietal Accreditation Commission has required since January 31, 2017 that all technical personnel in its accredited vascular laboratories hold a vascular credential. IAC also notes that CMS has imposed no nationwide accreditation mandate for vascular testing, though some state Medicare carriers require accreditation or sonographer credentialing for reimbursement.
What it costs, and how to get the number in writing
Cash prices published by MDsave, a marketplace that sells prepaid bundled procedures, put the estimated national average for an ultrasound of one body area at $401, against $221 for procedures booked through its own network, with listed prices running $116 to $495. Those two averages describe different things — the market rate and a negotiated rate — and MDsave publishes both on the same page rather than reconciling them. Venous Doppler studies run separately, listed at $155 to $359 for a unilateral study and $220 to $718 for bilateral. The urgent care visit fee is generally billed on top.
Federal law gives self-pay patients a mechanism to pin this down. Since January 1, 2022, the No Surprises Act has required providers to give uninsured and self-pay patients a written good-faith estimate itemized by CPT code, furnished within one business day when a service is scheduled at least three business days out, or within three business days when scheduled at least ten business days out or whenever a patient asks. If the final bill from that provider exceeds the estimate by $400 or more, CMS provides a patient-provider dispute resolution process, with 120 days to file and a $25 administrative fee at the time the rule took effect.
Four questions get a usable answer over the phone:
- Is ultrasound on site today, and does a clinician or a sonographer perform it?
- Is the study read at the bedside or sent out for a radiologist's report, and how long does that take?
- What is the self-pay price by CPT code, and does it include the visit fee and the interpretation?
- Can the written good-faith estimate come before the scan?
How long results take
For a bedside scan, there is no report to wait for. The clinician performing the scan is the clinician treating the patient, and the finding enters the visit in real time. That immediacy is the entire proposition.
For a study sent out, the wait is governed by policy rather than by a fixed national number. The ACR Practice Parameter for Communication of Diagnostic Imaging Findings sets no numeric deadline for routine reporting, requiring instead that non-routine and emergent findings be expedited and documented. A 2024 scoping review in the Journal of the American College of Radiology found the ACR parameter the most-cited guideline in the field, and noted that institutions commonly adopt the Massachusetts Coalition tiers of one hour, six to eight hours, and three days by urgency. Teleradiology vendors advertise tighter numbers. National Diagnostic Imaging publishes STAT reads normally within one hour and, in many cases, under 30 minutes. CMS tracks it formally through MIPS measure ACRAD15, which clocks mean hours from examination completion to final report signature.
Which questions still need a formal radiology study
A bedside scan that finds something has answered the question. A bedside scan that finds nothing has answered less than patients assume, which is the failure mode worth naming.
Send patients for a formal study when the calf veins are in question, or when a two-point compression scan comes back negative in someone with high clinical suspicion; when a first-trimester scan shows no intrauterine pregnancy, since a negative bedside look does not exclude ectopic pregnancy the way a positive one excludes it; when a complete organ survey is the point, such as a full abdominal protocol for gallbladder, liver, pancreas, kidneys and aorta; when anatomic detail or measurement precision drives the decision, including fetal dating and anomaly assessment; and whenever a documented duplex study with spectral waveforms is required for a vascular referral or a surgical decision.
At one Denver Health urgent care clinic, all ten providers completed a one-day POCUS course with a quality-assurance requirement of roughly 25 scans per application, and the before-and-after review ran in The Journal of Urgent Care Medicine. Transfers to the emergency department for soft-tissue and musculoskeletal complaints fell from 5.1 percent to 2.2 percent. For first-trimester pregnancy complaints, transfers fell from 28 percent to 20 percent, a difference that did not reach statistical significance. "Teaching UC providers POCUS skills resulted in a decreased percentage of patient transfers to the ED," wrote Lindsey E. Fish, MD, FCUCM, of Denver Health and the University of Colorado School of Medicine, with co-authors Genie Roosevelt, MD, and Amanda G. Toney, MD.
Even in a clinic that scans well, one in five pregnant patients still left for the emergency department.
